Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Ciscofanatics Everwell Health, Inc. is committed to protecting the privacy of your protected health information (“PHI”). This Notice applies to PHI created or received in connection with your care and is provided as required by the Health Insurance Portability and Accountability Act (HIPAA).

Our responsibilities

  • We are required by law to maintain the privacy of your PHI.
  • We must provide you with this Notice of our legal duties and privacy practices.
  • We must follow the terms of the Notice currently in effect.
  • We will notify you if a breach occurs that may have compromised the privacy or security of your PHI.

How we may use and disclose your PHI

  • Treatment: to provide, coordinate, or manage your healthcare with clinicians and other providers involved in your care.
  • Payment: to bill and obtain payment for services, including with your health plan.
  • Healthcare operations: for quality assessment, training, and administrative activities.
  • As required by law: for public health, safety, law enforcement, or other purposes permitted or required by law.

Other uses and disclosures — including most uses for marketing and any sale of PHI — require your written authorization, which you may revoke at any time.

Your rights regarding your PHI

  • Access: inspect and obtain a copy of your PHI.
  • Amendment: request corrections to your PHI.
  • Accounting of disclosures: request a list of certain disclosures we have made.
  • Restrictions: request limits on certain uses and disclosures.
  • Confidential communications: request that we contact you in a specific way or at a specific location.
  • Paper copy: obtain a paper copy of this Notice at any time.

Changes to this Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any we receive in the future. The current Notice will always be posted here.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
